Subcontractor Application

Please have your insurance company send a Certificate of Insurance naming Dallas 1 Corp. as the Certificate Holder and additional insured.
The amounts of insurance shall not be less than:
Worker’s Compensation – Florida Statutory Requirements (or a copy of your exempt)

Commercial General Liability:
$1,000,000 Limit each occurrence ($100,000  or less contract)
$2,000,000 Limit each occurrence ($100,001 to $1 mil contract)

Automobile Liability:
$1,000,000 Limit each occurrence ($1,000,000 or less contract)
(Not required if use of vehicles limited to driving to and from the job site.)

Employers Liability:
$1,000,000   Limit disease each employee  ($1,000,000 or less contract)
$1,000,000   Limit disease aggregate ($1,000,000 or less contract)
$1,000,000   Limit each accident ($1,00,000 or less contract)
